Name of product: Metal Halide Lamps

Hazard: The internal wiring can arc causing the lamp to catch fire or the glass to shatter. This poses fire and laceration hazards.

Remedy: Refund, Replace

Recall Details


Units: About 8,100

Description: The recalled items are egg-shaped, clear glass, 150-watt industrial metal halide lamps. They are about five inches long and have a medium base. Recalled lamps were manufactured between November 2012 and March 2013. "Phillips," "150W" "ALTO M142/0," "Hg," "USA 3A-1" and "MHC150/U/MP/4K" are stamped on the lamp. Each lamp comes in a protective cardboard sleeve. The Phillips logo appears at the top of the protective sleeve on all sides. "MasterColor," "Ceramic Metal Halide ED-17 Protected," "Hg – Lamp Contains Mercury" are on the side of the sleeve. "MHC150/U/MP/4K ALTO" and Universal Product Code (UPC) "46677 37724" are on a label on the side of the sleeve.

Incidents/Injuries: Philips has received one report of a failure that damaged a lamp. The company has received no reports of injury or property damage.

Remedy: Consumers should immediately stop using the lamps and contact Philips for a full refund or a replacement. Philips will issue a refund as a credit to all distributors with recalled lamps in their inventories and will replace recalled lamps that have been installed at no cost to the consumer.

Sold at: Professional electrical wholesale supply distributors for use by industrial and commercial property owners from November 2012 to April 2013 for about $20. The lamps have been installed in retail stores and recreational facilities.

Q: What is the problem with the product I have?
A: A quality and safety issue was recently discovered with respect to an inappropriate part that was specified and installed in the affected units during the time frame of November 15, 2012 – March 23, 2013.

Q: How can I tell if my product is affected?
A: In order to determine whether you have an affected unit, please check the part number and date of manufacture of all Philips' Metal Halide lamps you have on hand. This information can be found on both the packaging and on the product itself.

Q: My product appears to be operating okay. Can I continue to use it?
A: No. Even though the lamp appears to be operating correctly, we ask all customers to immediately stop using the product and participate in the recall by returning the product to us. All postage related to the return of this product will be paid by Phillips Lighting Company. We will in turn send you out, free of charge, new, good replacement product unaffected by the quality and safety issue.

Q: Can I remove and replace the product myself?
A: First, you must immediately stop using the product. Turn off the light switch prior to unscrewing and removing the bulb. Second, please contact Philips to take advantage of the recall. We will need you to return the product prior to receiving replacements. Please return all affected units to Philips Lighting Company, 7265 Route 54, Bath, NY 14810, Attn: Tanya Finney. Philips has arranged for you to ship the affected units, at no cost to you, via Federal Express using the following FedEx account number: 3567-9482-6. Once received and verified, Philips will create a replacement order, free of charge and send to your location.

Q: What if I am unable to remove and replace the product myself?
A: Yes. Philips will be happy to schedule a professional lighting service provider to visit your location in order to (a) uninstall the affected units; and (b) replace them with new, good product unaffected by the quality issue, free of charge.
